If you have booked an appointment for an inappropriate elimination issue with your cat, you may want to fill this form out ahead of time!

NOTE: Regardless of the method you use to submit the feline housesoiling form, please try to bring these ADDITIONAL items to your appointment: 1. A map of the location of the litter box(es) in your house AND 2. Photo(s) of the litter box area(s) in your house
3. Behaviour Consultation (based on Dr. Landsberg’s forms, these documents are used by us for case continuity in the event that a behaviour referral is required)
